How it works
Claim your listing, free. Tell us who you are. We confirm it with a call or email to the facility’s published number, usually within 2 business days.
Pick a plan, or stay on Free. Free keeps your verified listing, your responses beside Medicare findings, and the quarterly report.
Send your photos and details. Use the form below. We build your profile, you approve it, and it goes live, usually within 3 business days of receiving everything.
Families reach you through Care & Place. On Basic, family questions come to us and we help them reach you. On Premium, they go straight to your admissions team, routed through Care & Place so every inquiry is counted.
Billing and cancelling. Monthly plans renew on the 1st of each month. Cancel anytime: let us know by the 1st of the month, or that month is billed; your enhanced profile stays live until the end of the paid month. Premium annual is a 12-month subscription at $2,400 a year. Cancelling never removes your free listing.
Medicare ratings and inspection findings are shown the same way on every plan. Paying never changes them.
