Find care with confidence
Care & Place brings nursing homes and the information families need about them into one place — a single, nationwide directory built to make researching care simpler, clearer, and less overwhelming.
Why we exist
It usually starts the same way for every family: a phone call from the hospital, a doctor's appointment that didn't go the way anyone hoped, or a quiet realization at home that a parent or spouse needs more care than the family can safely provide alone. Suddenly there's a decision to make, and it has to be made soon.
What follows is often a scramble. A name scribbled down by a discharge planner. A search engine full of listings that all look the same. A handful of open tabs, none of them agreeing with each other, none of them built with an anxious family in mind. Somewhere in the middle of all that, the thing that actually matters — finding a good home for someone you love — gets harder than it needs to be.
Care & Place exists because researching care shouldn't be this hard. Families deserve one place where they can see the nursing homes near them, understand what sets them apart, and reach out with confidence — without having to become an expert in the industry first, and without wondering whether what they're looking at has been dressed up to look better than it really is.
What we built
The directory brings together every facility's identity, location, and essential information in one clear profile, so a family can get an honest first impression in minutes instead of hours. From there, the tools are built for how people actually make this decision: explore nearby homes to widen the search, compare a shortlist of facilities side by side, save the ones worth a second look, and reach out directly from a facility's own page when it's time to take the next step.
How it works, start to finish
A typical visit looks something like this: someone arrives not quite sure where to begin, types in a city or a ZIP code, and suddenly has a real, searchable list of homes in front of them instead of a blank page. From there, the pieces click into place one at a time.
- 1Search by name, city, or ZIP codeFind nursing homes near you, or anywhere in the country.
- 2Compare up to four facilities side-by-sideRatings, staffing, and quality measures, lined up next to each other.
- 3Save facilities as you researchKeep a shortlist in your browser and pick up where you left off, whenever you're ready to come back to it.
- 4Reach out directlyRequest more information from a facility right from its profile, once you've narrowed things down.
Who it's for
First and foremost, this directory is for families and caregivers researching a skilled nursing facility for a parent, spouse, or themselves — whether that's a plan being made months in advance or a decision that has to happen by the end of the week. Everything about how the site is built starts from that person's perspective: what would actually help someone in that moment.
Facilities can explore listing options and share information about their services. The goal is to help families understand their choices while keeping public CMS ratings separate from facility marketing.
How the directory works
Directory browsing is free. The site uses public CMS data to help families begin their research. Published ratings do not confirm current openings, insurance acceptance, or whether a home can meet a person’s specific care needs. Confirm those details directly with each facility.
Your first 15-minute call is free. Optional family navigation is offered as flat-fee packages, quoted before you decide, and we don’t accept referral fees from nursing homes. We also work with employers, attorneys and hospital teams; see For partners. Paid listing enhancements are separate from CMS ratings and do not change those ratings.
