We help families find and move into the right nursing home, independent of the facilities we recommend. Partners refer families to us or offer our navigation as a benefit, so the people they serve get experienced help at a hard moment.
How a referral works
- You point the family to usThey call, fill in our Get Help form, or, with their permission, you send us their name and number. No medical records are needed.
- We reply within one business dayA Care & Place family navigator contacts the family, Monday to Friday. Hospital discharges in the next few days go first.
- A free 15-minute callWe go over care needs, location, timeline and how the stay will be paid for, and explain the packages if the family wants more help.
- We do the legworkWe check Medicare ratings and inspections, call homes to ask about openings and payment, and give the family a shortlist. On a rush discharge, we work to the hospital’s 48–72 hour timeline.
- We keep you postedWith the family’s permission, we let you know when a home has been chosen, so your team can close the file.
